    Job Title: Detox Coordinator 

    Department: Detox 

    Reports To: VP of Operations 

    Location: TXM ER & Hospital - Manor 

    FLSA Status: Exempt 

    Employment Type: Full Time (1.0 FTE), salaried, rotating call 

    Shift Available: Business Hours 

    Schedule: Business hours, with flexibility to meet patient and program needs

    TXM ER & Hospital is building a high-accountability, patient-centered micro hospital platform designed to deliver prompt, high-quality care close to the communities we serve. We value team members who act with urgency, communicate clearly, take ownership, and work collaboratively across functions to solve problems quickly and safely. Our hospital has 6 ER beds, 6 Inpatient Beds, and 2 OR's. We have radiology, laboratory, dietary, and pharmacy services on site.

    Key Responsibilities

    • Coordinate the detox program end to end, from referral and intake through discharge and follow-up. 
    • Screen and assess prospective patients for program suitability, and escalate clinical concerns to the provider and nursing leadership without delay. 
    • Build and maintain each patient's individualized care and discharge plan in coordination with providers, nursing, pharmacy, and behavioral health. 
    • Serve as the primary point of contact for patients and families throughout the episode of care. 
    • Coordinate referrals and warm handoffs to outpatient treatment, counseling, and community recovery resources. 
    • Track program census, length of stay, completion, and readmission, and report trends to leadership. 
    • Partner with the Emergency Department on patients who present with withdrawal or substance use needs. 
    • Maintain program compliance with applicable state and accreditation requirements, including the confidentiality rules that apply specifically to substance use records. 
    • Support staff education on withdrawal recognition, de-escalation, and program referral pathways. 
    • Maintain patient safety, privacy, and regulatory compliance standards at all times. 

    Required Qualifications

    • Experience coordinating substance use, detox, or behavioral health services in a hospital, residential, or outpatient setting 
    • Demonstrated ability to assess patient needs, build care plans, and coordinate services across a multidisciplinary team 
    • Strong documentation skills and comfort working in an electronic medical record 
    • Ability to work with patients in crisis with professionalism, empathy, and appropriate boundaries 
    • Working knowledge of the confidentiality requirements that apply to substance use treatment records 

    Preferred Qualifications

    • Social work degree or equivalent behavioral health background 
    • Experience standing up a new program or service line 
    • Familiarity with community recovery and outpatient referral networks in the Manor and greater Austin area 
    • Bilingual English/Spanish 

    Physical and Work Environment Requirements

    • Work in both office and patient care settings 
    • Prolonged sitting for assessment, documentation, and coordination work, with periods of walking between areas 
    • Potential exposure to distressed, agitated, or combative patients 
    • Incidental exposure to blood, bodily fluids, and infectious disease in patient care areas, mitigated by PPE 
    • Requirement to use PPE and comply with infection control, safety, and regulatory standards 
    • Frequent keyboard and screen use requiring manual dexterity and visual acuity 
    • Ability to speak and listen effectively with patients in crisis and with their families 
    • Ability to remain composed and maintain appropriate boundaries in emotionally demanding situations 
    • Occasional lifting and carrying of supplies and materials up to 25 pounds
