Diabetes Education Program Quality Coordinator

Boulder Community Health is seeking a Diabetes Education Program Quality Coodinator who is responsible for operating and maintaining the Association of Diabetes Care and Education Specialists (ADCES) accredited program for Boulder Community Health (BCH). In collaboration with the Director of Integrated Clinical Services, the Coordinator oversees standards and quality guidelines of the program, supervises diabetes educators and staff, consults with BCH providers, and participates in quality improvement initiatives. The Program Quality Coordinator may also occupy the position of diabetes educator.

Qualifications

•Master of Science in nursing or nutrition preferred.

•Colorado RN (Registered Nurse) license in good standing or RD (Registered Dietitian) under the Commission on Dietetic Registration (CDR)

•Certified Diabetes Care and Education Specialist (CDCES) required for RDN or obtain within two years of hire for RN

•Active member of ADCES

•3+ years of clinical experience in ambulatory and/or inpatient care setting (i.e. primary care clinic, urgent care, case management, home health care, hospital, skilled nursing facility, or equivalent) required.

•3+ years of experience in an accredited diabetes education program required.

•1+ years of experience with diabetes-related medical devices preferred

•1+ years of supervisory experience preferred



Responsibilities

•Coordinates, supervises and is accountable for the daily activities of the ADCES accredited diabetes education program.

•Sets priorities for the team to ensure completion of tasks related to the diabetes education program.

•Impact of work is most often at team level.

•Owns output at task level: manages referrals, schedules and meets with patients, plans and leads group classes, documents in medical record, collects program data for required reporting and quality projects.

•Identifies and resolves operation problems using defined processes, expertise and judgment.

•Communicates clearly and effectively with physician, nursing staff, and patients regarding diabetes management.

•Organizes and prioritizes workload to most effectively accomplish goals.

•Facilitates behavior change by counseling patients / support person on informed lifestyle decisions and incorporating healthier choices in their diabetes self-management.

•Provides education and training on interventions that may prevent the development of diabetes.

•Motivates the patient to be more independent in decisions about treatment.

•Follows up with patients for ongoing behavior-change support.

•Consults with patients’ providers regarding medication and treatment changes

•Provides education to staff regarding diabetes disease process and treatments.

•Precepts dietetic interns, pharmacists, and nurses in their clinical experience

•Networks with colleagues and industry representatives

•Monitors program for compliance with ADCES accreditation. Maintains required records for program reporting: statistics, performance measures, program impact documentation and patient satisfaction.

•Submits re-accreditation application in a timely manner. Tracks submission through to final decision.

•Develops plans to meet short-term objectives

•Completes annual minimum of 15 hours of continuing education related to diabetes care and/or professional certification (program management, education, chronic disease care, and behavior changes).

•Decisions are guided by accrediting body, policies, procedures and business plan

•Product, service or process decisions are most likely to impact individual employees and/or customers (internal or external)
