What We Offer:

The Diabetes Education Specialist provides diabetes education, information, and training to patients, families, and team members. In addition, the Diabetes Education Specialist serves as a subject matter expert for the health system, participating in the design of diabetes care standardization and supporting system wide adoption of best practices. 

 

Work Schedule: NHRMC on-site Monday-Friday 8 am 5 pm, with virtual support to Brunswick Medical Center.

What We're Looking For:
  • Education: 4 Year / Bachelors Degree, required. BSN, required. Graduate Degree, preferred.
  • Experience: Minimum of three years of clinical experience in nursing, required.
  • Licensure/Certification: 
      • Registered Nurse in appropriate State, required.
      • Preferred- Certified Diabetes Education and Care Specialist (CDCES). If not certified, will require after one year. 
  • Additional Skills Required: Ability to successfully complete generic and department-specific skills validation and competency testing. Knowledge, ability and skills to support diabetes education to accommodate cultural diversity, age specific or behavioral needs among the patients being served. Ability to drive/travel to multiple locations/facilities as needed.
What You'll Do:
  • Provides diabetes education, information, and training to patients, families, and team members.
  • Assesses learning needs by evaluating and updating prior diabetes knowledge. Identifies barriers to care.
  • Provides diabetes survival skills education.
  • Assesses feasibility of medication management plans.
  • Instructs in use of diabetes medical equipment.
  • Assesses ability to obtain diabetes supplies and identify affordable options.
  • Identifes and mitigates risks factors for readmission.
  • Documents education and consultations in the medical record.
  • Assists with care coordination and transition to outpatient resources.
  • Communicates plan with patients and appropriate team members.
  • Provides diabetes education and training to clinical team members via classes, in-services, grand rounds, etc.
  • Consults with physicians and nurses regarding complex cases.
  • Provides trouble-shooting support regarding diabetes therapies, such as insulin pumps and continuous glucose monitors.
  • Assists nurses and physicians with glycemic management support during hospitalization.
  • Serves as a subject matter expert for the health system, participating in the design of diabetes care standardization and supporting system wide adoption of best practices.
  • Stays up-to-date with diabetes expertise through journals, local and national meetings, and professional organizations.
  • Participates in interdisciplinary teams to improve acute care diabetes outcomes.
  • Contributes to the development of clinical standards, policies, and procedures related to diabetes.
  • Assists with the evaluation of technology, equipment, and other products which support the management of diabetes.
  • Promotes evidence-based practice through research evaluation and utilization.
