The Certified Diabetes Care and Education Specialist (CDCES) ensures comprehensive multi-disciplinary care which is inclusive but not limited to, planning, administration and evaluation of care of patients with diabetes, and recommends and implements strategies to meet those needs. The CDCES is a resource to the physicians, nursing staff and works closely with the referring /covering endocrinologists to manage patient care. The CDCES is expected to participate in the Diabetes Task Force committee, quality improvement projects and staff education about inpatient hyperglycemia management.
CERTIFIED DIABETES CARE & EDUCATION SPECIALIST (NON-UNION)
Full-Time / Days to include one weekend a month
Responsibilities:
1.* Collects data and formulates assessments to identify the gaps and needs in the diabetes self-management of patients.
2.* Facilitates patient care through needs assessment based on all aspects of patient’s life and plans and implements new interventions accordingly.
3.* Able to download data from different meters, pumps and glucose sensors and able to interpret data and plan interventions accordingly.
4.* Develops educational goals, learning objectives and a plan for educational content and teaching methods to patients.
5.* Provides educational interventions that utilize primarily interactive collaborative, skill-based training methods.
6.* Coordinating diabetes resources for the patients, family members and staff.
7.* Provides accurate documentation of assessments, plans, interventions and evaluation of patient activities in the medical record including documentation in electronic medical file.
8.* Collaboratively develops an individualized follow-up plan with each patient.
9.* Evaluates effectiveness of educational services provided by measuring attainment of learning objectives through clinical and behavioral outcomes.
10.* Actively participates on an ongoing basis in the comprehensive therapeutic plan of the patient, is proactive in tailoring plan to specific patients or create specific interventions and communicate effectively with other healthcare providers as needed.
11.* Provides support to a multidisciplinary staff to ensure continuity of care for every patient on an individual basis. Utilizes team approach to provide services and communicate
12.* Acts as a consultant and resource person to other healthcare providers caring for diabetes.
13.* Participates in the development of training materials.
14.* Participates quality improvement process and identifies and addresses opportunities for improvement.
15.* Appraises his/her performance to identify areas of strength and areas for improvement and to develop a plan for growth and improvement.
16.* Performs all clinical and administrative responsibilities, as defined by the Director of the Endocrine Division and Chairman of the Department of Medicine.
17.* Attends all educational seminars and management rounds and all quality assurance and departmental meetings, as designated by the Director of the Division or Chairman of the Department of Medicine
18. * Maintains established departmental policies and procedures, objectives, quality assurance program safety, environmental, and infection control standards.
