Every service is delivered at no cost, regardless of insurance status.Role OverviewAs Diabetes Educator, you will teach seniors and families how to manage Type 1, Type 2, and pre‑diabetes through nutrition, glucose monitoring, medication management, and lifestyle changes. You will work in seniors' homes and via telehealth across LA County.Key ResponsibilitiesConduct comprehensive diabetes self‑management education (DSME) for newly diagnosed and existing patientsTeach blood glucose monitoring, insulin administration, and continuous glucose monitor (CGM) useDevelop personalized nutrition plans accounting for cultural preferences and food accessCoordinate with primary care, endocrinology, and home health teams on care plansLead group classes (in‑person and virtual) on diabetes prevention and managementDocument patient progress, A1C trends, and education milestones in our EHRQualificationsCurrent CDCES (Certified Diabetes Care and Education Specialist) credentialUnderlying RN, RD, or pharmacist license2+ years diabetes education or clinical experience with diabetic patientsStrong patient‑education skills; comfort with low‑literacy and non‑English speakersCultural humility around food, body image, and family dynamicsWhat we OfferCompetitive salary ($70,000–$95,000)Manageable caseload; home + telehealth visit mixCEU stipend; ADA national conference fundingComprehensive benefits including health, dental, visionMileage reimbursement for home visitsHow to ApplySubmit your application through the form on this page.
