Diabetes Self-Management Education (DSME) Registered Dietitian

About the Role
As a key member of our Diabetes Self-Management Education (DSME) team, you will design and deliver evidence-based educational, nutritional, and lifestyle programs for patients managing diabetes. Your work will directly support improved clinical outcomes, enhanced quality of life, and long-term patient success.
You will:
• Provide individualized nutrition assessments and develop tailored care plans for patients participating in the DSME program. • Lead engaging individual, group, and family education sessions focused on diabetes self-management. • Coach patients through lifestyle changes that support improved glycemic control and overall well-being. • Collaborate with interdisciplinary providers to ensure coordinated, patient-centered care. • Collect, validate, and analyze DSME program data to support continuous quality improvement.
Education Requirements
For Registered Dietitians completing didactic program after 1/1/24:
• Master's degree required • Bachelor's degree in Dietetics, Nutrition, or a related field
For Registered Dietitians completing didactic program before 1/1/24:
• Bachelor's degree in Dietetics, Nutrition, or a related field
Experience
Minimum five years of clinical experience required. Prior teaching or patient-education experience in a specialty area preferred. Demonstrated critical thinking, clinical competence, and expertise. Experience providing outpatient services and collaborating with diverse provider disciplines.
Licensure & Certification
Active Registered Dietitian (RD/RDN) licensure in Maryland. Certified Diabetes Educator (CDE) preferred or eligibility to obtain certification.
Knowledge, Skills & Abilities
• Strong understanding of CPT and ICD coding relevant to outpatient diabetes education • Familiarity with American Diabetes Association (ADA) standards for recognized DSME programs • Commitment to ongoing professional development and excellence in practice • Exceptional communication, coaching, and patient-engagement skills • Ability to work independently while contributing to a collaborative team environment
