Exemption Status: ExemptHiring Range: $62,004.80 - $111,633.60Schedule Details: Monday through FridayScheduled Hours: day shift flexibleShift: 1 – Day Shift, 8 Hours (United States of America)Hours: 40Cost Center: 10013 – 4290 Leominster Multi‑ClinicSigning bonus available (eligibility confirmed with Recruitment Team during interview).Under the direction of the Manager, Diabetes Education Program, the Diabetes Educator will be responsible for the assessment, planning, implementation and evaluation of Diabetes Self‑Management Education and Support (DSMES) plans for patients and families with diabetes. The role collaborates with primary care providers and diabetes clinicians to develop and implement diabetes self‑management education plans.Major ResponsibilitiesUtilizes patient education principles to assess, plan, implement and evaluate comprehensive DSMES plans for patients with diabetes.Conducts an assessment of the patient's knowledge base of diabetes and readiness and motivation to learn. Identifies barriers to learning including age, physical, cognitive, environmental, cultural, language, financial, psychological and spiritual needs.Based upon assessment, designs an individualized DSMES plan based on the patient and multidisciplinary teams mutually agreed upon goals.Promotes continuity of care by working collaboratively with all members of the multidisciplinary team in helping patient achieve diabetes goals.Utilizes adult teaching‑learning methods and a variety of presentation styles to provide education to patients and families.Position QualificationsLicense/Certification/EducationBachelor's Degree from an Accreditation Council for Education in Nutrition and Dietetics (ACEND) approved four‑year program in Food and Nutrition or related field.Certified Diabetes Educator (CDE) credential required within two years of hire.Maintains a license with the Board of Dietitians and Nutritionists in the Commonwealth of Massachusetts.Maintains registration with the Commission on Dietetic Registration.PreferredMaster's degree.Experience/SkillsRequiredMinimum of two years diabetes‑related experience.Ability to influence the quality of patient care in an advisory capacity to the physician and other health‑care team members.Demonstrated skill in analyzing problems and making decisions.Oral and written communication skills are essential to interact with patients, staff, and referring providers.PreferredDiabetes education experience in outpatient setting.Experience with EMR.Unless certification, licensure or registration is required, an equivalent combination of education and experience providing proficiency in the areas of responsibility listed may be substituted for the above requirements.Physical Demands and Environmental ConditionsWork is considered sedentary.
