Please note: NSORNA Nisqually's Sex Offender Registration and Notification Act requires all individuals who have ever been convicted of a sex offense to register with our Public Safety Department. This is true whether or not you have to register with another jurisdiction.This position has regular contact with or control over Indian Children and Vulnerable Adults. In compliance with Federal Law, this position will not be filled by an applicant with any felonious offense or any of two or more misdemeanor offenses under Federal, State, or Tribal law involving crimes of violence; sexual assault, molestation, exploitation, contact, or prostitution; crimes against persons; or offenses committed against children. NATURE OF WORK: The Diabetes Program Educator is responsible for diabetes prevention activities, educational resources, nutrition services, and program support overseen by the Diabetes Coordinator. Activities in the Diabetes Program are for meeting all objectives of applicable Diabetes grant deliverables, public health policy and priorities of the Nisqually Tribal Health Center. The Diabetes Program Educator will serve as a navigator for diabetes patients helping them understand medical tests, healthy eating and lifestyle.  DUTIES AND RESPONSIBILITIES:Participate as a member of a team in support of the Diabetes Program.Perform work to achieve grant and program deliverables such as: chart review, patient contact through letters, phone and other means, and data collection to improve data accuracy and community outcomes. Participate in meetings and trainings either required by applicable grants or that facilitate grant or program activities such as meeting with local and regional staff, specific skills in using IT systems for SDPI, or Indian Health Service sponsored training designed to assist local tribes in performing SDPI activities. Provide one-on-one and small group Medical Nutrition Therapy in the Nisqually health clinic and complete documentation of visits for persons at risk for or diagnosed with diabetes.Provide outreach and education to the tribal community regarding services available for diabetes prevention and treatmentPrepare for and host events to draw community members into discussion and learning about diabetes prevention and treatmentCollaborate with other programs within the community to provide education to promote practices that prevent diabetesDraft monthly content for reports, newsletter articles related to diabetes such as: promote nutrition services and practices that prevent diabetesProvide nutrition counseling, weekly menu planning, shopping assistance and other duties at the Adult healing house per request.Coordination of diabetes events and activities within budget guidelines.Assist Diabetes Coordinator with coordinating diabetes program services within the Department and ensure appropriate Electronic Health Record and data retention.Perform other duties as assigned. The duties and responsibilities described are not a comprehensive list and additional tasks may be assigned to the employee.KNOWLEDGE, SKILLS AND ABILITIES:Knowledge of Tribal, Local, State and Federal resources available to program clients and their families.Knowledge of Indian Health Service programs.Knowledge of Health Department, Tribal, and other related policies, procedures, and systems.Knowledge of HIPAA and other laws, rules, and regulations related to the security and confidentiality of data.Knowledge of electronic health records, systems, and related applications.Knowledge of accounting and budgeting principles.Skill in gathering and interpreting data.Skill in maintaining an organized work environment.Skill in both verbal and written communication.Ability to take on responsibility and exercise independent judgment, decision making, and problem solving expertise.Ability to research and prepare reports or other correspondence as required.Ability to demonstrate compassion and be proactive in assisting patients with their healthcare needs.Ability to provide excellent customer service and exercise tact and good judgment in all contacts.Ability to work with diverse teams of people.Ability to multi-task and manage priorities. MINIMUM QUALIFICATIONS:Registered Dietitian or Certified Nutritionist.2 years of recent related work experience in diabetes nutrition services or a closely related field.OR a comparable combination of approved education and/or recent related work experience. Must have a valid driver's license and be insurable under the Tribe's insurance policy.
