Setting: Long-Term Care (LTC) / Skilled Nursing Facility – Dialysis Den
Schedule: Monday/Wednesday/Friday (MWF) or Monday/Wednesday (MW), with additional shifts available


Position Overview


We are seeking an experienced Dialysis Registered Nurse (RN) to join our team providing hemodialysis services in a dedicated Dialysis Den within a Long-Term Care / Nursing Home setting.


Both full-time and part-time opportunities are available. Candidates may work a consistent Monday/Wednesday/Friday (MWF) schedule or Monday/Wednesday (MW) schedule, with opportunities to pick up additional shifts.


The Dialysis RN is responsible for providing safe, high-quality hemodialysis treatments while supervising Patient Care Technicians (PCTs), coordinating with nursing facility staff and the nephrology team, and maintaining compliance with clinical, infection-control, and regulatory requirements.


Key Responsibilities


  • Supervise, mentor, and provide clinical direction to Patient Care Technicians (PCTs).
  • Perform and supervise direct patient care for hemodialysis, including pre-treatment assessment, initiation, monitoring, termination, access hemostasis, and post-treatment evaluation.
  • Complete nursing assessments and accurately document patient condition, treatment progress, and outcomes.
  • Assess patients' response to hemodialysis and make appropriate adjustments or recommendations in accordance with physician orders.
  • Assess and monitor target/dry weights and fluid status.
  • Administer dialysis-related medications as ordered.
  • Review, transcribe, and implement physician orders.
  • Monitor dialysis adequacy and support vascular access management and catheter-reduction initiatives.
  • Monitor and review laboratory results and coordinate required laboratory testing and follow-up.
  • Ensure proper specimen collection, correct laboratory tubes, labeling, packaging, and shipment.
  • Promptly report critical, panic, or abnormal laboratory results to the physician.
  • Recognize and respond appropriately to dialysis-related complications, emergencies, and adverse events.
  • Communicate significant changes in patient condition to the nephrologist, Medical Director, nursing home staff, and dialysis management.
  • Maintain competency in emergency procedures and ensure required emergency equipment is available and operational.
  • Maintain compliance with infection prevention, patient safety, and dialysis policies and procedures.
  • Participate in routine infection-control and safety audits.
  • Assist with patient scheduling and coordination of dialysis treatments.
  • Collaborate with nursing home staff, nephrologists, dietitians, social workers, PCTs, and other members of the interdisciplinary team.
  • Educate patients and families regarding ESRD, dialysis therapy, vascular access, fluid management, diet, medication compliance, treatment modalities, and transplant awareness.
  • Participate in care conferences, QAPI activities, staff meetings, and ongoing education.
  • Perform other duties as assigned.
Minimum Qualifications


  • Current and unrestricted Registered Nurse (RN) license.
  • Minimum of one year of dialysis nursing experience required.
  • Current BLS certification.
  • Demonstrated competency in hemodialysis treatments and vascular access management.
  • Ability to recognize and appropriately respond to dialysis-related complications and emergencies.
  • Strong nursing assessment, documentation, organizational, and communication skills.
  • Ability to supervise PCTs and provide clinical leadership during dialysis treatments.
  • Ability to work independently while collaborating effectively with the interdisciplinary care team.
Preferred Qualifications


  • Previous experience in a Long-Term Care, Skilled Nursing Facility, or Dialysis Den setting.
  • Experience caring for medically complex and elderly dialysis patients.
  • Familiarity with dialysis laboratory monitoring, QAPI, infection-control requirements, and regulatory standards.
