Sunset Nursing and Rehabilitation Center is looking to add a Dietitian to its team. In this role, you’ll help ensure residents receive individualized nutritional care that supports their health, well-being, and quality of life. RESPONSIBILITIES:





+ Complete and oversee nutritional assessments for newly admitted residents and develop individualized nutrition care plans



+ Monitor resident weights, meal intake, supplements, hydration status, laboratory values, and therapeutic diet needs



+ Collaborate with the Food Service Director, Diet Technician, nursing staff, therapists, and other disciplines to ensure appropriate nutritional care



+ Provide nutrition counseling and education to residents, families, and facility staff



+ Review menus, recipes, dietary consistencies, and nourishment programs to ensure nutritional adequacy and regulatory compliance



+ Participate in care plan meetings, interdisciplinary meetings, and quality improvement initiatives



+ Maintain required documentation, transfer/discharge summaries, and nutrition-related records



+ Assist with policy review, dietary education programs, and maintaining the facility diet manual



+ Perform additional duties as assigned





REQUIREMENTS:





+ Minimum education requirement: Associate's Degree in Dietetics/Nutrition; Bachelor's Degree in Dietetics/Nutrition preferred



+ Minimum experience requirement: Ten full-time equivalent years of education and supervised experience (1,600 hours of experience equals one year of credit for every 30 semester hours of education); completion of Bachelor's program experience requirements preferred



+ Valid NYS Dietitian License required



+ Strong knowledge of nutritional assessment, care planning, hydration management, therapeutic diets, and food service operations



+ Excellent communication, education, and interdisciplinary collaboration skills



+ Strong organizational, leadership, and problem-solving abilities



+ Ability to maintain regulatory compliance, confidentiality, and professional standards
