Job Summary
The Direct Care Worker provides compassionate, hands-on support to clients in their homes, assisting with personal care needs, daily living activities, and promoting a safe, healthy environment. This role emphasizes fostering client independence, dignity, and well-being.

Key Responsibilities

Follow the plan of care to ensure safe, competent, and effective support for clients.
Assist clients with personal hygiene tasks, including bathing, grooming, and toileting.
Support clients with ambulation and mobility as instructed by the care plan.
Plan and prepare nutritious meals and shop for groceries, as directed.
Perform light housekeeping tasks to maintain a clean and comfortable living environment.
Encourage clients to participate in activities that promote mental alertness and independence.
Provide emotional support and build trusting relationships with clients and their families.
Monitor and report changes in clients' mental or physical condition to the supervisor.
Accurately complete and submit daily visit documentation in a timely manner.
Attend in-service training sessions to maintain compliance with agency requirements.

Qualifications

High school diploma or equivalent preferred.
Successful completion of one of the following training programs:
- A minimum of 75 hours of training and competency evaluation as specified in 42 CFR 484.80.
- An agency-developed training program meeting PA Code 611.55 criteria.
- A competency evaluation program that meets 42 CFR 484.80 requirements.
- A nurse aide training and competency evaluation program approved by the state.
Knowledge of personal care, household management, and meal preparation.
Understanding and respect for client rights, ethics, and confidentiality.
Current CPR certification (online certifications not accepted).
Successful criminal background check.
Reliable transportation is preferred; bilingual proficiency is a plus.
