Position Overview:The Direct Service Provider / Life Skills Coach supports individuals with disabilities by promoting independence, safety, and meaningful participation in their communities. This role provides person-centered support through daily living assistance, community engagement, skill development, and documentation of services. Team members build positive relationships while helping individuals achieve their personal goals and maintain a high quality of life.
