The Contracted Direct Service Worker (DSW) role is a flexible, community-based opportunity for professionals interested in delivering impactful mental and behavioral health support services. This is an independent contractor position with service opportunities across several programs, including PRP, TBS and Care Coordination.

DSW’s who work in Care Coordination will work with minors age 5-17 to enroll; once enrolled may continue through age 21. There are 3 levels of Care Coordination: Level 1 is base level and clients receive 3 hours of services monthly, with at least 30 minutes in person. Level 2 is 7.5 hours of services monthly, with at least 1 hour in person. Level 3 is 15 hours of services monthly, with at least 1.5 hours in person. Level 3 referrals come only from LBHA. Clients enrolled in Levels 2 and 3 are able to be enrolled in both CC and PRP. Wrap around support services including referrals to community resources and supports including PRP, TBS, and other supports, advocacy, and more.

Compensation & Structure:
This is a 1099 contractor role. Compensation is task-based and paid upon completion of required services, documentation, and training. A full breakdown of the compensation structure will be provided during onboarding. You will have an opportunity to make between $30-$35/hour based on services completed.

 

Direct Services:

  • Conduct comprehensive assessments using the Child and Adolescent Needs and Strengths assessment (CANS).
  • Responsible for implementing Level I, II, or III care coordination services, based on assessed needs.
  • Develop and implement client-centered Care Plans.
  • Facilitate Child and Family Team (CFT) meetings.
  • Provide Care Coordination and advocacy services, including but not limited to, referrals, entitlement applications, service coordination, housing, and transportation support.
  • Assist clients with developing and maintaining social support networks
  • Monitor client engagement in services
  • Connect clients to behavioral and somatic health care services in the community
  • Able to juggle multiple families/youth, documentation, scheduling, and deadlines.
  • Comfortable communicating with clients, caregivers, providers, and internal team members.
  • Able to assess needs, prioritize services, and creatively problem-solve barriers to care.
  • Can navigate EHR systems, maintain accurate documentation, and understands the importance of timely documentation and compliance with program standards
  • Willing to work with multiple systems (schools, courts, therapists, etc.) and contribute to a larger care team.
  • Deep understanding of trauma-informed care and working with diverse, underserved populations.
  • Support families in navigating systems such as housing/food insecurity, education/IEPs/504 plans, Legal/juvenile systems and 
  • Has community connections (already knows local resources and providers)
  • Passion for long-term growth and leadership in the organization

 

Documentation:

  • Maintain accurate and up-to-date client records.
  • Document all interactions, assessments, and care plans according to agency standards.
  • Submit all documentation by specified deadlines and communicate any difficulties in doing so. 

 

Coordination:

  • Coordinate with community resources and service providers.
  • Advocate on behalf of clients to ensure their needs are met.
  • Collaborate with each client’s various treatment providers on a regular basis.

 

Supervision:

  • Participate in monthly supervision sessions 
  • Attend a company staff meeting every quarter
  • Attend a general staff meeting on a quarterly basis.
  • Attend clinical training sessions when required.

 

Qualifications:

  • Bachelors in social work, psychology, public health, or related field
  • Direct experience in care coordination, case management, or wraparound services is a plus.
  • Familiarity with youth and family systems, behavioral health, and community resource navigation
  • Familiarity with Child and Adolescent Needs and Strengths assessment (CANS).
  • Familiarity with Child and Family Team (CFT) meetings
  • Reliable and current auto insurance (State minimum for Maryland).
  • Valid Maryland Driver’s license.
  • Active Professional Liability Insurance.
  • CPR/ First Aid Certification.
