Provide direct 1:1 support based on the individuals service plan, goals, and preferences. Implement person-centered plans with a focus on independence and community inclusion. Support daily activities including personal care, communication, and behavioral support as neededFacilitate participation in community-based and meaningful day activities. Accurately document all services in accordance withOE standards and regulatory requirements.Monitor and report progress toward goals. Ensure health, safety, and well-being at all times.
