Direct Support Professional

D Square Healthcare Services Inc is looking for a compassionate and dedicated Direct Support Professional to join our team in Belle Plaine, MN. Responsibilities:

• Provide direct care and daily living support to individuals with intellectual or developmental disabilities
• Assist clients with personal hygiene, grooming, meal preparation, and household tasks
• Implement individualized support plans (ISPs) and document progress accurately
• Transport and accompany clients to medical appointments, community activities, and social outings
• Administer medications and follow health care protocols as directed
• Foster independence, dignity, and community inclusion for each individual served
• Communicate effectively with supervisors, family members, and healthcare professionals

Requirements:

• High school diploma or GED equivalent required
• Prior experience in direct support, caregiving, or human services preferred
• Valid driver's license and reliable transportation
• Ability to pass a background study through the Minnesota Department of Human Services (DHS)
• CPR and First Aid certification (or willingness to obtain)
• Strong communication, patience, and problem-solving skills
• Ability to work flexible shifts, including evenings, weekends, or overnights
