Direct Support Professional

Service individuals with physical assistance if needed, assist with personal hygiene, daily living skills, making beds, doing laundry, preparing meals, washing dishes, ironing, cleaning the individual’s personal space, etc.

· Provides assistance with individuals purchasing and preparing meals and groceries, and assistance with grocery shopping.

· Engage the individual by reading, talking, listening to the individual

· Assist the individual with doctor appointments, and community outings that the individual would like to participate in.

· Provides assistance with other various tasks, as directed or upon request by individual, family, or guardian.

· Completes service notes on every shift that you service an individual

· Completes monthly notes, once a month for the individual you provide services to

· Initiates activities with individuals which include educational, leisure and social interactions.

· Assist with meal preparation, serving, and cleaning

· Administer medication reminders.

· Immediately document, and report all individual crisis and incidents.

· Provide assistance with daily activities in individuals home or in the community

· Document daily progress notes in full detail daily.

· Participates in multi-disciplinary treatment team meetings.

· Attend all mandatory staff meetings and mandatory trainings that are required by the state and County (6 meetings a year)

· Report and document all non- emergency issues to the supervisor.
