Director Case Management Bradenton, FL

A Director of Case Management is sought for a hospital in Bradenton, FL. This is an executive-level leadership role with full operational accountability for the case management department, including care coordination, discharge planning, and clinical resource utilization across the facility.

The role. Reporting to senior facility leadership and working in close partnership with the CFO, CMO, and CNO, the Director oversees all daily case management operations, including staffing models, skill mix, FTE allocation, onboarding, and staff competency development. The Director drives adherence to care coordination and discharge planning workflows, ensuring timeliness, documentation quality, and regulatory compliance. A significant focus of the position is performance improvement — analyzing operational and outcomes data, identifying gaps, and implementing sustainable solutions. The Director also cultivates relationships with ancillary service leaders and external agencies to support continuity of care and optimal clinical resource utilization.

What we're looking for.

• Bachelor's degree required
• Master's degree in Nursing, Health Administration, or Business Administration preferred
• Active licensure as a Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) required
• 3+ years of acute care hospital case management experience required
• 2+ years of case management leadership experience required

Schedule. Full-time, day shift, Monday through Friday with no weekend obligation.

Compensation. $109,762 - $164,570 per year plus Sign on: $10,000 for non-HCA Employees Relocation Assistance: Yes 17.5% annually bonus (1st payment 2027)
