Healthcare United is seeking a Lead Physician to guide clinical teams in delivering patient-centered, value-based care. This role combines direct patient care, leadership development, and strategic partnership to redefine how healthcare is delivered.


    Job Description

    As the Lead Physician, you will:

    • Deliver exceptional primary care to a panel of medically complex members.
    • Lead an interdisciplinary care team of physicians, nurses, behavioral health specialists, medical assistants, care managers, and community health workers.
    • Coach and mentor providers on value-based care delivery, chronic disease management, and effective team leadership.
    • Provide clinical leadership by answering clinical inquiries, guiding care plans, and consulting on complex cases.
    • Partner with the Medical Director to develop and implement clinical protocols, collaborate with hospital systems, payers, and community organizations, and attend leadership meetings.

    Key Responsibilities

    Clinical Leadership & Care Delivery:

    • Deliver comprehensive primary care and manage acute and chronic conditions onsite.
    • Provide urgent care appointments for members recently discharged from hospitals or skilled nursing facilities.
    • Manage care transitions, including hospital discharges, ER visits, and follow-ups.
    • Track and meet quality metrics and care outcomes using evidence-based guidelines.
    • Participate in on-call rotations with fellow providers.

    Team Development & Mentorship:

    • Mentor and support Advanced Practice Providers and physicians in developing clinical skills and care leadership.
    • Facilitate population health huddles and guide teams toward achieving clinical and utilization goals.
    • Develop clinical protocols and pathways to enhance the skillsets of clinical staff.

    Strategic Partnerships & Community Engagement:

    • Partner with community-based organizations, academic programs, and healthcare systems to expand services.
    • Serve as Healthcare United’s ambassador, representing the organization to payers and stakeholders.


    Requirements

    • Active, unrestricted medical license in the state of employment.
    • 5+ years of clinical experience in primary care, community health, or hospital-based care.
    • Experience leading interdisciplinary teams in clinical or administrative settings.
    • Strong expertise in chronic care management, value-based care models, and population health.
    • Proven success in mentorship, team leadership, and healthcare service delivery improvement.
    • Exceptional problem-solving, project management, and interpersonal communication skills.
