What you will do:
The Director of Care Coordination manages day-to-day operations of the Care Coordination department. Oversee patient care activities of the department, ensuring smooth transition of care for patient within Peterson Health. Responsible for providing leadership to all staff, ensuring quality care and managing change. Carries out all duties while maintaining compliance, confidentiality, and promoting the mission and philosophy of the organization.

Bachelor of Science in NursingÂ

Master of Science in Nursing or Master of Business Administration
Required Skills:
Leadership experience, basic computer and keyboarding
