Certifications/Licenses:
- BLS
- Director of Care Coordination
- Must meet the following qualifications:
- BSN required, master's degree in nursing, Health Administration or Business Administration is preferred.
- Case Management certification (e.g., CCM or ACM) preferred.
- Minimum 5 years of experience in an acute healthcare setting required.
- Minimum 5 years of progressively responsible leadership/administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function required.
- Demonstrated experience leading interdisciplinary clinical/operational programs, staff, performance improvement, and regulatory or accreditation readiness.
- Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care environment.
- Day Shift, Monday - Friday
