Overview

This leadership role provides the opportunity to guide care management teams in delivering patient‑centered care across both hospital and ambulatory settings. You will drive strategic initiatives to improve patient outcomes, optimize healthcare resources, and strengthen collaboration among clinical and community partners. This position is ideal for a leader passionate about advancing care management strategies and making a meaningful impact on healthcare delivery.

Key Responsibilities

• Provide strategic leadership and operational oversight for Integrated Care Management programs across hospital and ambulatory settings
• Lead multidisciplinary teams including RN Care Managers, RN Care Coordinators, and Licensed Social Workers
• Build strong partnerships with clinical leaders, community organizations, and cross‑functional stakeholders
• Drive process improvement initiatives to reduce readmissions, improve patient flow, optimize length of stay, and enhance care coordination outcomes
• Ensure compliance with federal and state regulatory requirements
• Coach, mentor, and develop team members while fostering collaboration, accountability, and service excellence
• Analyze clinical, operational, and financial performance metrics to support strategic growth and quality improvement

Required Qualifications

• Bachelor's degree or higher
• 1+ years of Care Management experience
• 1+ years of experience in ANY of the following:
  • Sunrise Patient Financials Analytics
  • Clinical Quality
• ANY of the following valid licenses/certifications:
  • Registered Nurse (RN) – Iowa (IA)
  • Licensed Clinical Social Worker (LCSW) – Iowa (IA)
• ANY of the following valid licenses/certifications:
  • Licensed Independent Clinical Social Worker (LICSW)
  • Licensed Independent Social Worker (LISW)
  • Licensed Clinical Social Worker (LCSW)

Schedule & Shift Details

• On‑site work only

Pay Transparency Compensation varies based on location, experience, and performance. Some sites require disclosure of pay and benefits. This role may also be eligible for bonuses or incentives depending on business needs.

The estimated hourly rate for this position is $120,000.00
