This is a high-impact leadership opportunity for an experienced Case Management leader to oversee operations across a large, high-acuity acute care hospital. You will take ownership of care coordination, discharge planning, and resource utilization across a complex, fast-paced environment, driving outcomes through strong interdisciplinary collaboration and data-driven performance improvement.
Overview
Reporting to the VP of Case Management, this Director will lead all aspects of Case Management, Social Work, and Chaplain services across a 790-bed, Level II Trauma facility. This is a visible leadership role responsible for aligning clinical and operational strategy, optimizing patient flow, and ensuring high-quality, compliant care coordination across the continuum.
What You Will Do
- Provide strategic and operational leadership for Case Management, Social Work, and Chaplaincy services
- Oversee day-to-day operations including staffing, workflows, and performance management across ~110 FTEs
- Lead and develop a leadership team (4 Managers, Social Work Supervisor, Administrative support)
- Drive care coordination and discharge planning processes to ensure timely, compliant, and patient-centered outcomes
- Partner closely with executive leadership, physicians, and post-acute providers to improve throughput and resource utilization
- Analyze operational and clinical data to identify gaps and implement sustainable performance improvement initiatives
- Ensure compliance with regulatory standards, policies, and best practices
- Collaborate with CFO and system leadership on departmental goals, financial performance, and strategic initiatives
- Support a high-performing culture focused on accountability, communication, and continuous improvement
What You Bring
- Bachelor's degree required; Master's preferred (Nursing, Healthcare Administration, or Business)
- RN, LCSW, or LMSW required
- 3+ years of acute care hospital Case Management experience
- 2+ years of leadership experience in Case Management
- Proven ability to lead large, complex teams and drive measurable outcomes
- Strong analytical skills with the ability to translate data into actionable plans
- Executive presence with excellent communication and relationship-building skills
- Experience collaborating with physicians and post-acute networks
