Position Overview

Tap Healthcare Solutions is seeking an experienced Director of Case Management to lead case management operations for a respected healthcare organization in Mayfield, Kentucky. This senior leadership opportunity is designed for a Registered Nurse with strong case management expertise who is ready to guide teams, strengthen care coordination, and help ensure patients receive appropriate, efficient, and well-coordinated care throughout their healthcare journey.

In this role, you will provide leadership for case management activities while promoting effective interdisciplinary collaboration, regulatory compliance, resource utilization, and successful transitions of care. Offering a competitive salary of $95,000 – $125,000 annually, this position provides an opportunity to combine clinical expertise with organizational leadership and make a meaningful impact on both patient outcomes and healthcare operations.

Required Qualifications

  • Active and unrestricted Registered Nurse (RN) license.
  • Professional experience in Case Management.
  • Accredited Case Manager (ACM) certification through ACMA, or ability to obtain the required certification within two (2) years of hire.
  • Strong understanding of care coordination, utilization management, discharge planning, and transitions of care.
  • Excellent communication, critical thinking, organizational, and problem-solving skills.
  • Ability to collaborate effectively with physicians, nursing leadership, social services, and other members of the interdisciplinary care team.

Preferred Qualifications

  • Previous leadership experience within Case Management.
  • Experience supervising, mentoring, or developing clinical case management teams.
  • Knowledge of healthcare regulatory requirements, utilization practices, and quality improvement initiatives.
  • Experience supporting departmental performance, workflow improvement, and patient-centered care initiatives.
  • Demonstrated ability to build strong working relationships across clinical and administrative departments.
