A bit about us:

We are representing Encompass Health Rehabilitation Hospital of Parkersburg, a 50-bed inpatient rehabilitation hospital seeking an experienced Director of Case Management to lead its hospital-based Case Management department.

The hospital operates at consistently high occupancy, with an average daily census of approximately 49 patients, and patients typically remain in the hospital for approximately 7-14 days.

This is a true on-site leadership position reporting directly to the hospital CEO and supported by the Regional Director of Case Management.

The Case Management department currently includes full-time, PRN, and support staff across both nursing and social work backgrounds. The organization is seeking a leader who is comfortable being physically present with the team, developing staff, solving problems in real time, and taking ownership of Case Management performance across the hospital.

Job Details

The Director of Case Management is responsible for leading the hospital's Case Management function and ensuring effective utilization review, discharge planning, interdisciplinary coordination, patient progression, and safe transitions of care.

The department currently consists of:

4 full-time Case Managers

1 full-time Case Management Assistant

2 PRN Case Managers

A mix of RN and Social Work professionals

Case Managers generally maintain caseloads of approximately 12-15 patients each.

The Director does not typically carry an individual patient caseload. However, this is a hands-on leadership position, and the Director is expected to understand the team's patients, support complex cases, and assist with patient coverage when staffing or operational needs require it.

The CEO is seeking a leader who has demonstrated stability in previous roles and meaningful experience directly supervising Case Management professionals in an on-site healthcare setting.

Responsibilities

Lead the hospital's Case Management department and directly supervise Case Management staff
Oversee discharge planning, utilization review, patient progression, and care coordination
Manage and develop a multidisciplinary team of RN and Social Work Case Managers
Maintain visibility into active patient cases and assist with complex discharge barriers
Partner with physicians, nursing, therapy, hospital leadership, patients, families, and external providers
Ensure safe, timely, and effective discharge planning
Monitor departmental performance and Case Management workflows
Coach staff, address performance concerns, and build accountability within the department
Support regulatory compliance and established documentation standards
Assist with patient coverage when staffing needs arise
Partner directly with the CEO and Regional Director of Case Management on operational priorities
Promote strong communication and collaboration across hospital disciplines

Qualifications

Minimum 3 years of hospital-based Case Management experience
Demonstrated experience with Utilization Review and Discharge Planning
Previous Case Management leadership experience strongly preferred
Must be qualified to independently complete assessments within the scope of the candidate's professional discipline
If professional licensure is available and required for the candidate's discipline, an active applicable license is required
RN candidates must possess a Bachelor of Science in Nursing
RN candidates must hold active RN licensure
Other eligible healthcare professionals must possess at least a bachelor's degree
Graduate degree preferred for non-nursing candidates
Social Work candidates may be considered, including appropriately licensed clinical Social Workers
Candidates with a master's degree in an appropriate healthcare-related discipline may also be considered based on experience and scope
Strong understanding of hospital discharge planning, utilization review, and interdisciplinary care
Demonstrated ability to lead employees directly in an on-site healthcare setting
