Director of Coordination

$120,000 - $140,000

A growing home care agency is seeking an experienced Director of Coordination to oversee all care coordination operations. This leadership role is responsible for ensuring seamless patient coverage, managing coordination teams, improving operational efficiency, and maintaining compliance with New York State home care regulations.

Responsibilities

• Lead and manage Care Coordinators and Coordination Supervisors, ensuring high performance and accountability.
• Oversee daily staffing to ensure all authorized patient cases are fully covered.
• Manage callouts, emergencies, overtime, and difficult-to-staff cases.
• Oversee live-in and split cases while ensuring compliance with labor laws and company policies.
• Partner with HR, Payroll, Recruitment, Nursing, Intake, Authorizations, and Retention teams to resolve staffing challenges.
• Ensure accurate documentation of scheduling changes, coordination activities, and case notes.
• Handle caregiver and patient scheduling concerns and implement solutions when needed.
• Monitor key performance metrics, including staffing coverage, overtime, response times, and complaints, and develop improvement plans.

Requirements

• 5+ years of leadership or management experience within a home care agency.
• At least 2 years of hands-on home care coordination experience.
• Strong knowledge of New York State home care regulations and labor requirements.
• Experience managing high-volume coordination teams in a fast-paced environment.
• Experience using HHA Exchange.
• Excellent leadership, communication, and problem-solving skills.
• Experience overseeing multi-region or high-census coordination teams is highly preferred.
