Salary Range: $80,000 - $110,000

Position Overview
Our well established client is seeking an experienced Director of Home Care to lead and advance its community-based care programs. This senior leadership role is responsible for overseeing home care and adult day services, ensuring high clinical standards, operational strength, and continued program growth.
This opportunity is ideal for a Registered Nurse who brings both strong clinical expertise and the leadership vision to guide teams, enhance care delivery, and expand services that help individuals remain safely in their homes and communities.
Key Responsibilities
Program Leadership & Strategy
- Provide overall direction and leadership for home and community-based services
- Support program growth, sustainability, and operational performance
- Partner with executive leadership to align home care services with broader organizational goals
Clinical Oversight & Compliance
- Ensure delivery of safe, high-quality patient care in accordance with regulatory and professional standards
- Maintain compliance with all applicable state and federal home care requirements
- Monitor program performance and implement quality improvement initiatives
Team Leadership & Development
- Supervise, mentor, and support clinical and non-clinical staff
- Lead hiring, onboarding, and ongoing training initiatives for caregiving personnel
- Foster a positive, collaborative, and accountable team culture
Program Innovation & Collaboration
- Identify opportunities to improve service delivery and patient outcomes
- Collaborate with internal departments and external partners to enhance continuity of care
- Contribute ideas and solutions to address evolving home care challenges
Qualifications
- Active New York State Registered Nurse (RN) license required; BSN preferred
- Minimum of 5 years of leadership or supervisory experience in home care or community-based healthcare
- Strong knowledge of home care regulations, standards, and best practices
- Demonstrated ability to lead teams and manage complex programs
- Excellent communication, organizational, and problem-solving skills
- Passion for improving patient care and supporting individuals in community settings
Preferred:
- Experience with waiver programs such as Nursing Home Transition and Diversion (NHTD) or Traumatic Brain Injury (TBI)
- Background in caregiver training and program development
