    Director of Nursing (DON) – Home Health

    Millennium Healthcare | Full-Time | Monday–Friday | $160,000 - $200,000/year

    We are looking for an experienced Director of Nursing (DON) to lead our home health clinical operations and help shape the future of our clinical team.     Position Summary

    The Director of Nursing provides leadership and oversight of all home health clinical services. Reporting directly to the Executive Director/Administrator, the DON is responsible for clinical quality, patient care, regulatory compliance, staff development, and the overall effectiveness of the nursing department.

    The ideal candidate is an experienced home health leader who can balance clinical excellence, team leadership, compliance, and operational performance while creating a supportive and accountable environment for clinicians.

    What You'll Do

    Clinical Leadership & Patient Care

    • Lead and oversee the delivery of high-quality home health services.
    • Provide clinical guidance and support to Clinical Supervisors, nurses, therapists, and other clinical personnel.
    • Ensure appropriate assessment, care planning, implementation, and evaluation of patient care.
    • Monitor clinical quality, patient outcomes, and overall standards of care.
    • Ensure patient services are delivered safely, effectively, and in accordance with applicable regulations and organizational policies.
    • Review and oversee clinical documentation and patient records for accuracy, completeness, and compliance.

    Team Leadership & Development

    • Lead, mentor, and support the clinical leadership team.
    • Participate in the recruitment, hiring, onboarding, performance management, and retention of clinical staff.
    • Conduct and oversee performance evaluations and provide constructive feedback and coaching.
    • Identify opportunities to improve clinician performance, productivity, and patient care.
    • Develop and oversee orientation, onboarding, in-service education, and ongoing clinical training programs.
    • Foster a collaborative, accountable, and positive clinical culture.

    Compliance & Quality

    • Ensure compliance with applicable federal, state, and local requirements governing home health services.
    • Maintain readiness for regulatory surveys and accreditation requirements, including applicable Joint Commission standards.
    • Participate in the organization's Quality Assurance/Performance Improvement (QAPI) activities.
    • Monitor clinical indicators, identify trends, and implement corrective or improvement initiatives when needed.
    • Develop and maintain clinical policies and procedures in collaboration with organizational leadership.
    • Stay current with changes in home health regulations, clinical standards, and best practices.

    Operations & Strategic Leadership

    • Partner with the Executive Director/Administrator on clinical and operational planning.
    • Monitor census, referral volume, staffing needs, productivity, and other operational indicators.
    • Assist with departmental budgeting, resource planning, and expense management.
    • Ensure adequate availability of clinical supplies and equipment.
    • Collaborate with referral sources and healthcare partners to support appropriate home health referrals and strengthen community relationships.
    • Assist leadership in developing and achieving organizational goals.
    • Serve as the clinical leader of the organization in the absence of the Executive Director/Administrator, as assigned.
    • Perform other duties as needed to support the organization and its patients.

    What We're Looking For

    Required Qualifications

    • Current Registered Nurse (RN) license in the state of practice.
    • Bachelor's degree in Nursing (BSN) from an accredited program.
    • Minimum 3 years of management experience in home health or a related healthcare setting.
    • At least 1 year of supervisory, administrative, or clinical leadership experience.
    • Demonstrated home health experience is required.
    • Strong knowledge of home health clinical operations, regulations, documentation, and quality standards.
    • Excellent leadership, communication, organization, and problem-solving skills.
    • Ability to effectively lead and develop clinical teams.
    • Valid driver's license, reliable transportation, and automobile insurance meeting applicable requirements.

    Preferred Qualifications

    • Master's degree in Nursing, Healthcare Administration, or a related field.
    • Experience as a Director of Nursing, Clinical Director, or Clinical Manager in home health.
    • Experience with accreditation, regulatory surveys, QAPI, and performance improvement initiatives.
    • Experience managing multidisciplinary clinical teams.
