Director of Nursing-LTC

A registered nurse who assumes full responsibility for the supervision of the nursing staff of the Skilled Nursing/Long Term Care center on a 24-hour basis; leading the nursing team ensuring the delivery of high-quality, resident-centered care. The Director of Nursing oversees nursing staff and ensures regulatory compliance

ESSENTIAL FUNCTIONS:

• Manages nursing employees in the provision of care and services to residents
• Supervises, trains and evaluates nursing staff, hire and onboard nursing staff; make recommendations regarding recruitment, hiring, discipline, performance, evaluation, orientation, training and direction of nursing staff
• Develops staffing plans to assure sufficient number of qualified, competent nursing staff to meet resident care needs, ensuring target staff coverage is met
• Establish criteria to ensure services provided meet the established standards of quality
• Supervises and develops in-services education programs designed to equip nursing staff with adequate knowledge and skills to provide nursing care to each resident
• Perform regular performance reviews, take appropriate personnel actions, review job performance, ensuring staff meet professional standards and performs all essential functions of their respective jobs
• Perform as charge nurse if needed; maintain current skills and knowledge through continuing education and apply information to job responsibilities
• Keeps Administrator and Chief Nursing Officer informed of reportable situations and nursing unit needs
• Assist in coordinating admission, room assignments, and discharge of residents
• Ensure a registered nurse screens resident for goal directed therapy soon after admission or re-admission according to procedures
• Participates in and monitors actual nursing care; This includes on-call emergency availability.
• Develops nursing policies and procedures that reflect current clinical practice, professional standards, and facility mission, vision and values of care delivery
• Communicate and interpret policies and procedures to nursing staff; monitor for effective implementation
• Compliance with and communication to staff, facility policies and procedures for workplace safety, infection control procedures, application of universal precautions for blood borne pathogens, use of personal protective equipment and handling of hazardous materials
• Communicate emergency and disaster procedures of facility
• Patient care and treatment rendered, medication pass observation, medication chart audits
• Ensure provision of nursing and nursing related services to maintain the highest practicable, physical, mental and psychosocial well-being of residents
• Participate in medical reviews; direct or provide staff to direct care needs of residents
• Conduct resident assessments as required, developing care plans, evaluating residents' responses to interventions and documenting records efficiently and effectively in compliance with tribal, state, federal requirements
• Responds to emergent medical situations
• Communicate with residents, their families, medical staff, nursing staff, interdisciplinary team members and other departments leaders to coordinate care and services
• Reviews the 24 hours report to ensure timely, effective responses to significant changes in condition, transfers, discharges, use of physical or chemical restraints, unexplained injuries, potential or suspected abuse or neglect, medication errors, loss of resident property, or any evidence of resident or family dissatisfaction, keeping Administrator informed
• Develop methods for coordination of nursing services/care with other resident services to ensure the continuity of care
• Ensure safety of all residents, visitors and staff
• Report unsafe conditions immediately
• Ensure staff attends required training
• Serves on the quality committee
• Assures nursing staff attend required training in hazardous communication and trainings related to providing care to residents, fire safety training,
• Provides fiscal management for department operations
• Plans, schedule, supervise, and inspect nursing schedules to ensure target staffing is met
• Follows established procedures/guidelines for maintenance of storage of supplies and equipment
• Purchase equipment and supplies as necessary or required; purchases may need to be preapproved
• Implements, coordinates and monitors nursing quality management activities to include, but not limited to:
• Performs chart audits for completeness, accuracy, and timely documentation, as well as ensuring proper judgment is exercised in relation to treatment and care plan, and expeditiously address any discrepancies, inaccuracies or errors
• Coordinate and conduct quality assessment and assurance activities, including regulatory compliance rounds to monitor performance and to continuously improve quality
• Gather and analyze facts, assess situation, investigate, notify appropriate parties, resolve issues, and record facts properly for suspect and confirmed events
• Design and manage continuous quality improvement initiatives
• Actively participate in long term care survey process; take corrective action while survey is in progress if appropriate, collaborate with Administrator to develop responses to survey report as needed, and prepare and educate staff on regulations so organization is survey ready at all times
• Performs other related duties as assigned by the Facility Administrator and/or other relevant leadership
• Five (5) years of recent relevant and demonstrated progressive leadership experience as a Nurse Manager or other relevant supervisory experience in a long-term care setting, or long-term care director of nursing certification required
• Bachelor's in Nursing required; Master's Degree in Nursing, preferred
• Current and active state unrestricted RN license, Arizona state license preferred
• BLS and ACLS certification required
