Core Responsibilities


  • Provide strategic and operational leadership for all clinical services, ensuring the delivery of safe, high‑quality, patient‑centered home health care in compliance with federal, state, and accreditation standards.
  • Oversee the intake, review, and clinical appropriateness of all case referrals by evaluating patient information, required disciplines, and care complexity to determine service needs.
  • Direct clinician assignment and staffing decisions to ensure optimal patient outcomes, staff utilization, and regulatory compliance.
  • Lead and manage all clinical staff, including hiring, onboarding, training, competency validation, performance evaluation, and retention strategies.
  • Establish and oversee structured onboarding and hands‑on field training programs for newly hired clinicians to ensure clinical proficiency and regulatory readiness.
  • Perform and oversee OASIS assessments and field visits as necessary to support quality assurance, risk mitigation, and operational continuity.
  • Maintain accountability for clinical excellence by reviewing cases through documentation audits, case conferences, record reviews, and direct collaboration with clinical staff.
  • Guide clinicians in interpreting clinical findings, validating impressions, and strengthening clinical decision‑making to improve patient outcomes and survey readiness.
  • Ensure continuous availability of clinical leadership during operating hours to support clinicians with complex cases, urgent concerns, and care coordination needs.
  • Review and evaluate patient diagnoses, medications, procedures, and clinical progression to ensure accurate care planning and compliance with standards of care.
  • Lead the development, implementation, and oversight of individualized plans of care, ensuring alignment with patient goals, physician orders, and regulatory requirements.
  • Chair or actively participate in interdisciplinary case conferences to promote collaboration, continuity of care, and quality improvement.
  • Drive recruitment efforts by leading or supporting clinician interviews, selection decisions, and onboarding recommendations in alignment with organizational needs.
  • Oversee clinician orientation programs and ensure staff competency with agency policies, procedures, clinical protocols, and documentation standards.
  • Ensure full compliance with professional practice standards, Conditions of Participation (CoPs), state regulations, and agency Standard Operating Procedures.
  • Uniformly enforce agency policies and procedures, modeling accountability, consistency, and professionalism across the clinical team.
  • Lead by example in alignment with the agency’s Four Core Tenets, fostering a culture of integrity, excellence, accountability, and patient advocacy.
  • Implement ongoing competency assessments and performance monitoring to ensure clinicians’ adherence to policies, procedures, and evidence‑based practice.
  • Conduct joint field visits with clinicians to provide mentorship, reinforce clinical best practices, and support continuous quality improvement.
  • Collaborate closely with the Administrator and executive leadership to support operational goals, survey readiness, risk management, and organizational growth.
  • Assume responsibility for additional clinical and administrative initiatives as delegated by the Administrator.
QUALIFICATIONS


  • Registered nurse with an active license and able to practice in Florida state.
  • Two years of experience in a home health is mandatory 
  • Well organized, and have excellent verbal and communication skills.
  • Experience using any EMR (we use Synergy).
  • Knowledge of AHCA regulations and CHAP standards.
  • Knowledge of OASIS
