Director of Nursing (DON)

Transitions Healthcare Washington, PA is seeking an experienced, compassionate, and results-driven Director of Nursing (DON) to lead our nursing team and oversee all clinical operations within our skilled nursing facility.

The Director of Nursing is responsible for providing strategic leadership, ensuring the delivery of exceptional resident care, maintaining regulatory compliance, and fostering a positive, collaborative work environment. This individual will work closely with the interdisciplinary team to promote clinical excellence, staff development, and a culture of accountability, respect, and continuous improvement.

The ideal candidate is a strong clinical leader with extensive long-term care experience, exceptional communication skills, and a passion for developing and supporting nursing professionals while delivering outstanding care to residents and their families.

Key Responsibilities

  • Provide leadership, direction, and oversight for all nursing services and clinical operations.

  • Ensure the delivery of high-quality, resident-centered care that meets or exceeds federal, state, and local regulatory requirements.

  • Develop, implement, and monitor nursing policies, procedures, and quality improvement initiatives.

  • Recruit, mentor, coach, and retain a high-performing nursing team.

  • Foster a positive workplace culture that emphasizes teamwork, accountability, professional growth, and resident satisfaction.

  • Collaborate with physicians, department leaders, therapists, and other interdisciplinary team members to coordinate resident care.

  • Monitor staffing levels and scheduling to ensure appropriate clinical coverage while maintaining budgetary goals.

  • Oversee infection prevention and control practices in accordance with current regulations and evidence-based standards.

  • Lead survey preparation efforts and maintain ongoing survey readiness.

  • Investigate incidents, identify trends, and implement corrective action plans as needed.

  • Monitor clinical outcomes and quality measures, utilizing data to drive performance improvement.

  • Ensure accurate and timely clinical documentation and compliance with reimbursement requirements.

  • Promote effective communication with residents, families, physicians, and team members.

  • Maintain compliance with all company policies, CMS regulations, Pennsylvania Department of Health requirements, and applicable accreditation standards.

Qualifications

  • Current Registered Nurse (RN) license in good standing in the Commonwealth of Pennsylvania.

  • Minimum of 3–5 years of progressive nursing leadership experience in a skilled nursing or long-term care setting.

  • Previous experience as a Director of Nursing, Assistant Director of Nursing, or comparable clinical leadership role preferred.

  • Thorough knowledge of federal and state regulations governing long-term care facilities.

  • Proven ability to lead, motivate, mentor, and develop nursing staff.

  • Strong clinical assessment, organizational, problem-solving, and decision-making skills.

  • Excellent verbal and written communication skills.

  • Demonstrated ability to manage multiple priorities in a fast-paced healthcare environment.

  • Proficiency with electronic health records; PointClickCare experience preferred.

  • Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing, Healthcare Administration, or a related field preferred.
