Overview
We re seeking an experienced Registered Nurse (RN) to lead our home health clinical team as Director of Patient Care Services. In this role, you ll drive high-quality patient care, ensure regulatory compliance, and support a collaborative, patient-centered environment.
Responsibilities
- Lead and supervise all home health clinical operations and staff
- Ensure quality patient care in accordance with physician orders and care plans
- Maintain compliance with Medicare, Medi-Cal, and state regulations
- Oversee staff hiring, training, performance, and scheduling
- Drive performance improvement and ensure accurate clinical documentation
- Coordinate interdisciplinary care and serve as a key liaison with physicians
- Manage patient admissions, discharges, and overall care delivery
Minimum Qualifications:- Registered Nurse with current licensure in the practicing state
- Current CPR certification
- Minimum of two years of home health experience
- Minimum of two years of clinical leadership, management, or supervisory experience preferred
