Focus Home Health is seeking an experienced and compassionate Director of Patient Care Services (DPCS) to lead our San Jose clinical team. RESPONSIBILITIES:

• Coordinate and oversee all direct and indirect patient services
• Supervise clinical staff effectively and complete performance actions
• Identify and report any unusual occurrence, accidents or adverse effects of medical devices
• Assist with follow up review and documentation
• Provide leadership in the assessment of patients and their families
• Take referrals and ensures that assessment visits are made
• Responsible for the efficient assignment, scheduling and supervision of clinical staff
• Review medical records routinely and conduct clinical record reviews to supervise the quality and timeliness of care and documentation.
• Investigate adverse events to identify opportunities to improve care

QUALIFICATIONS:

• A registered nurse with three years of experience in a home health agency with experience in a supervisory or administrative capacity
• Excellent interpersonal, verbal and written communication skills and ability to recognize the needs and concerns of people and maintain constructive working relationships with them
• Must have a vehicle, valid driver’s license and automobile insurance
• Ability to make home visits and provide skilled nursing assessments and care in the home environment
