Pay Ranges: $130000.00 - $150000.00 per year (Based on Experience)  

Bonus for Full-Time Position: $4,000 Hiring & Retention Bonus (Limited Time Offer) 


Why This Role Matters 

We are seeking a licensed Physical Therapist, Occupational Therapist, or Speech-Language Pathologist to serve as Director of Rehabilitation. This leadership role is responsible for overseeing the rehabilitation program, mentoring a multidisciplinary therapy team, and ensuring the delivery of high-quality, resident-centered care in alignment with regulatory and organizational standards. 

Reporting: This position collaborates closely with facility leadership, nursing, physicians, and interdisciplinary teams to support clinical excellence and operational performance. 

Role Overview 

  • Lead and manage the daily operations of the rehabilitation department 
  • Supervise, mentor, and support Physical Therapists, Occupational Therapists, Speech-Language Pathologists, assistants, and therapy aides 
  • Ensure compliance with federal, state, and facility regulations, including documentation standards 
  • Collaborate with nursing, physicians, and interdisciplinary teams to coordinate resident care 
  • Develop and oversee individualized care plans and support PDPM processes 
  • Monitor productivity, clinical outcomes, and departmental performance metrics 
  • Participate in audits, care conferences, and leadership meetings 
  • Promote a positive, collaborative, and professional work environment 


What We’re Looking For 

  • Active California license in Physical Therapy, Occupational Therapy, or Speech-Language Pathology, in good standing 
  • Minimum of one year of experience as a Director of Rehabilitation in a skilled nursing facility is required 
  • Bachelor’s degree in Physical Therapy, Occupational Therapy, or Speech-Language Pathology (or higher) 
  • Prior leadership experience within a rehabilitation department preferred 
  • Strong knowledge of PDPM, Medicare, and skilled nursing facility regulations 
  • Experience with EMR systems and regulatory compliance 
  • Excellent leadership, communication, and organizational skills 
  • Ability to foster teamwork, professional growth, and clinical excellence 
  • Passion for serving the geriatric population
