Director of Rehab

Transitions Healthcare Gettysburg is currently seeking an experienced Director of Rehab to lead and oversee rehabilitation services at our skilled nursing community.

As Director of Rehab, you will lead an interdisciplinary rehabilitation team focused on providing high-quality, resident-centered care. We're looking for a collaborative leader who can support and develop the therapy team while maintaining strong clinical, operational, and regulatory standards.

Director of Rehab – Essential Job Functions

• Plan, organize, and manage the effective and efficient delivery of rehabilitation services within the facility.

• Lead and develop an interdisciplinary rehabilitation team while fostering a collaborative and supportive work environment.

• Supervise department personnel, including orientation, training, scheduling, and performance evaluations.

• Review and maintain department policies and procedures to ensure compliance with facility standards and state and federal regulations.

• Conduct monthly quality assurance reviews, including chart audits of active and discharged residents, and ensure identified deficiencies are addressed.

• Conduct quarterly program evaluations, identify opportunities for improvement, and develop appropriate action plans.

• Prepare weekly and monthly management reports addressing therapy productivity, indirect time analysis, and associated action plans.

• Review referrals and discharges from rehabilitation and Long-Term Care restorative programming to evaluate service delivery and resident needs.

• Participate in facility meetings as needed.

• Partner with facility leadership on rehabilitation marketing efforts, referral screenings, community and referral-source relationships, program outcomes, and opportunities for new rehabilitation programs.

• Identify staff education needs and provide or coordinate appropriate in-service education.

• Provide direct resident care when needed and appropriate.

• Attend required in-service and educational programs.

• Assist in the evacuation of residents during emergency situations.

• Perform other duties as directed by the Administrator.

Director of Rehab – Minimum Qualifications

• Current Pennsylvania license in good standing as a PT, PTA, OT, COTA, or SLP.

• Minimum of three (3) years of rehabilitation experience.

• Previous rehabilitation leadership or supervisory experience preferred.

• Strong leadership, communication, organizational, and problem-solving skills.

• Proficiency with Microsoft Office Suite and Outlook.

• PointClickCare experience preferred.

• Ability to read, write, and understand the English language.

• Ability to meet the physical requirements of the position, with reasonable accommodations available for qualified individuals with disabilities.
