We're looking for a Director of Social Services who brings compassion, organization, persistence, and a little bit of sunshine to the lives of our residents and their families.

In this role, you'll be more than a resource—you'll be an advocate, listener, problem-solver, planner, and trusted partner for residents, families, and our interdisciplinary team.
What You'll Do

  • Build meaningful relationships with residents and families while helping establish goals and a path forward.
  • Help residents achieve their highest practicable level of self-care, independence, and well-being.
  • Provide medically related social services that support each resident's physical, mental, and psychosocial well-being.
  • Listen, assess, and identify social, emotional, psychological, and family needs.
  • Develop and maintain care plans addressing social services needs.
  • Provide compassionate emotional support during difficult times, including health challenges, changes in ability, loss, grief, and end-of-life situations.
  • Help residents and families navigate healthcare decisions with confidence and understanding.
  • Connect residents with community, social service, health, and welfare resources.
  • Assist with personal shopping needs and help track resident belongings.
  • Coordinate responses when belongings are missing, lost, or stolen.
  • Help residents obtain appropriate prosthetics, assistive devices, and other needed resources.
  • Schedule outpatient appointments and coordinate on-site services such as optometry, podiatry, dentistry, and psychiatric services.
  • Participate in discharge planning by coordinating with agencies, equipment providers, referral sources, residents, families, and the interdisciplinary team.
  • Maintain strong partnerships with facility departments and community agencies.
  • Accurately document residents' social service status and related interventions.
  • Provide in-service training to staff when requested.
  • Counsel and support residents and family members through challenging situations.
  • Bring persistence, professionalism, confidentiality, objectivity, and proactive problem-solving to the role every day.

What You'll Need

Education & Experience

  • Bachelor's degree in Social Work or Human Services preferred
  • 2 years of supervised social work experience in a healthcare setting working directly with individuals preferred

Communication & Reasoning
  • Ability to read and understand technical procedures and policies.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to solve practical problems and interpret written, verbal, diagram, and schedule-based instructions.
  • Ability to apply basic mathematical concepts, including fractions, percentages, ratios, and proportions, to practical situations.

Preferred Experience
  • Knowledge and experience with PCC preferred.
Supervision

This position has no supervisory responsibilities.

Pay Rate $23.00-$28.00
