Key Responsibilities
- Oversee the delivery of services through the Social Service Department.
- Organize and monitor Social Service programs to ensure quality and outcomes.
- Develop departmental resident care policies.
- Function as the Discharge Planning Coordinator for the facility.
- Stay current with State and Federal Long Term Care regulations.
- Participate in meetings with the Administrator and weekly Department Head meetings.
- Engage in special projects as requested by the Administrator.
- Ensure compliance with Health Information Privacy (HIPPA) laws and regulations.
- Assist residents and families during the admissions process and in understanding Advanced Directives and payment sources.
- Provide supervision to staff social workers.
- Collaborate with Administration and other disciplines in the development of programs and policy regarding Social Services and resident care.
Requirements
- Bachelor's or master's degree in social services or a related field.
- Licensed Social Worker (LSW) in the Commonwealth of Massachusetts.
- Two to four years of experience working with the geriatric population, preferably in long-term care.
Preferences
- Licensed Independent Clinical Social Worker (LICSW) preferred.
