Navigate the Journey: From the moment of admission to a successful discharge, you will guide patients and families through transitions, providing clinical support and expert resource coordination.

Build the Dream Team: You will recruit, mentor, and inspire a team of dedicated social workers (or manage the function individually in specialized centers), fostering professional growth and excellence.

Key Responsibilities
*Clinical Excellence: Conduct deep-dive psychosocial assessments, develop interdisciplinary care plans, and provide therapeutic interventions for residents adjusting to long-term care.

*Strategic Administration: Manage departmental budgets, staffing needs, and Quality Improvement initiatives. You’ll be the key Social Services voice in Morning Meetings and Utilization Management.

*Discharge Mastery: Coordinate seamless transitions back to the community by building strong linkages with behavioral health, hospice, and other community-based providers.
