This highly visible role serves as the community's expert and champion for all Veterans receiving services through the Veterans Affairs Community Nursing Home (CNH) Program. The Director of Veteran Care Services will build and lead our Veteran-focused care strategy, strengthen relationships with Veterans Affairs partners, and ensure every Veteran receives compassionate, personalized, and high-quality care.

Position Overview

The Director of Veteran Care Services serves as the primary liaison between Heights of Bulverde and Veterans Affairs representatives while supporting excellence in Veteran-centered care delivery throughout the community.

In partnership with the Administrator, Director of Nursing Services, Social Services team, interdisciplinary departments, and VA stakeholders, this leader will oversee care coordination, regulatory compliance, quality assurance initiatives, Veteran satisfaction, and CNH Program requirements.

What You'll Do

Lead Veteran Care Excellence

• Serve as the community's subject matter expert for the VA Community Nursing Home (CNH) Program.
• Promote a culture of Veteran-centered, trauma-informed care across all departments.
• Champion Veteran rights, dignity, individual preferences, and personalized care goals.
• Support implementation of Veteran-focused wellness programs, recognition events, and community engagement initiatives, including Touchstone's Heroes Among Us program.

Coordinate Veteran Admissions & Transitions

• Review and evaluate Veteran referrals prior to admission.
• Partner with interdisciplinary teams to assess clinical, behavioral, psychosocial, and service needs.
• Coordinate smooth admissions, transitions, and discharge planning processes.
• Ensure admission readiness across nursing, therapy, dietary, activities, social services, and leadership teams.
• Conduct admission audits and verify completion of Veteran-specific care plans and preferences.

Build Strong VA Partnerships

• Serve as the primary point of contact for VA Program Managers, Case Managers, Social Workers, CNH representatives, and PACT Teams.
• Maintain collaborative relationships that support positive outcomes for Veterans.
• Ensure timely communication regarding admissions, hospitalizations, discharges, changes in condition, and other required notifications.
• Coordinate specialty services, appointments, transportation, and authorized care.

Drive Quality & Regulatory Compliance

• Monitor compliance with VA CNH requirements, company policies, and applicable regulatory standards.
• Conduct chart audits, quality reviews, and documentation assessments.
• Review physician documentation, care plans, nutritional assessments, and monthly VA requirements.
• Assist with VA site visits, audits, surveys, and compliance reviews.
• Track Veteran-specific quality indicators and identify opportunities for process improvement.

Advocate for Veterans & Families

• Conduct routine quality rounds and meet regularly with Veterans and family members.
• Participate in care conferences and interdisciplinary planning meetings.
• Address concerns proactively and assist with grievance resolution.
• Provide education regarding VA benefits, services, and community resources.
• Monitor discharge outcomes and post-transition follow-up activities.

Support Clinical Operations & Risk Management

• Participate in administrative nursing on-call responsibilities.
• Support nursing leadership with clinical decision-making and emergent operational needs.
• Participate in investigations involving Veteran-related incidents and concerns.
• Assist with reporting requirements involving falls, injuries, hospitalizations, abuse allegations, significant changes in condition, and grievances.
• Support corrective action plans and ongoing performance improvement initiatives.

What We're Looking For

Required Qualifications

• Current Texas LVN or RN license in good standing.
• Minimum of one year of experience in skilled nursing or long-term care and working with Veterans or VA programs
• Working knowledge of healthcare regulations, documentation standards, and clinical compliance requirements.
• Strong organizational, communication, and relationship-building skills.
• Ability to effectively collaborate with interdisciplinary teams, families, and external partners.

Preferred Qualifications

• Knowledge of VA Community Nursing Home (CNH) Programs.
• Experience with Medicare, PDPM, discharge planning, and care transitions.
• Leadership, quality assurance, performance improvement, or clinical program management experience.
