We are seeking multiple Directors of Rehabilitation, one opportunity near Halifax, Virginia.
· Short term and Long term Rehab services
· Established Rehab team
Qualifications and Responsibilities
· Valid and current license as a therapist or therapist assistant granted from the state of employment (PT/PTA/OT/COTA/SLP)
· Previous supervisory experience
· Previous experience in Long-Term Care (LTC), Skilled Nursing (SNF) setting
· Preferred knowledge of Patient Driven Payment Method (PDPM), but not required
· Participate in community planning, discharge planning, development and implementation of social care plans and resident assessments.
· Interview residents and families to obtain social history.
· Refer residents and families to appropriate social service agencies if needed.
· Provide information regarding Medicare/Medicaid, and other financial assistance programs available to the resident.
