Job Description

The Wellness Director is responsible for the overall leadership and oversight of clinical and personal care services within the community. This includes ensuring high-quality care, regulatory compliance, effective staffing, and interdepartmental collaboration to promote residents’ health and well-being. The Wellness Director plays a key leadership role in enhancing resident outcomes, fostering a culture of safety and compassion, and supporting the community’s mission and values.

The position requires a Registered Nurse (RN) license in the state of California. Exceptionally experienced Licensed Vocational Nurses (LVNs) may be considered.

Job Responsibilities

 

Leadership & Oversight:

·         Lead the Wellness Department, ensuring clinical operations are organized, proactive, and aligned with community standards and regulations.

·         Oversee the coordination of healthcare services, including third-party providers (e.g., home health, hospice, rehabilitation).

·         Participate actively in leadership meetings and serve as a liaison between residents, families, and care providers.

·         Develop, implement, and regularly review individualized care plans for residents.

·         Monitor and assess resident health status and adjust care plans as needed.

·         Establish and enforce documentation standards and ensure timely charting.

Staff Management & Training:

·         Hire, onboard, supervise, and evaluate Wellness team members, including medication technicians, caregivers, and shift leads.

·         Develop and maintain staffing schedules to ensure 24-hour care coverage and compliance.

·         Provide ongoing coaching and training on care protocols, regulatory standards, and best practices.

·         Oversee orientation, in-service education, and mandatory training to ensure staff competency and compliance.

Regulatory Compliance & Risk Management:

·         Ensure the community complies with all RCFE (Residential Care Facility for the Elderly) regulations, HIPAA privacy standards, and infection control guidelines.

·         Participate in audits, surveys, and prepare reports for the Department of Social Services and other governing bodies.

·         Monitor for clinical incidents, respond appropriately, and ensure timely documentation and reporting.

·         Maintain clinical licensure verifications for wellness staff in coordination with HR.

Resident Engagement & Advocacy:

·         Foster trusting relationships with residents and their families through regular check-ins, care plan reviews, and availability for consultation.

·         Participate in Resident Council meetings to address wellness concerns and answer health-related questions.

·         Ensure wellness services are personalized, respectful, and centered on the dignity and autonomy of each resident.

Operational Excellence:

·         Oversee wellness supply inventory, ordering, and budgeting in alignment with community resources.

·         Collaborate with the Executive Director and Business Office on the annual department budget and monthly financial reviews.

·         Respond to after-hours emergencies and participate in the Manager on Duty (MOD) rotation.

·         Make frequent rounds to ensure quality of care, environmental safety, and compliance across all shifts.

Qualifications

QUALIFICATIONS:

·         Graduate of an accredited nursing program.

·         Current RN license in the state of California (LVNs may be considered with significant experience and demonstrated leadership).

·         Minimum 2 years of experience in geriatric or assisted living nursing; leadership or supervisory experience preferred.

·         RCFE Administrator Certification preferred.

·         In-depth knowledge of RCFE regulations and California Title 22.

·         Strong written and verbal communication skills.

·         Proficient in basic computer software (e.g., Microsoft Office, electronic health records).

Proven leadership skills with a team-oriented and resident-focused approach.
