• Letter(s) of Recommendation (Three (3) signed and dated within the last three years.) OR Letter(s) of Reference (Three (3) signed and dated within the last three years. )
• Other (EL Authorization (if embedded in credential, evidence of such is required))
• Resume
• Pupil Personnel Services Credential - School Psychology OR Pupil Personnel Services Credential - School Social Work
