Responsibilities:
  • Provide direct support and assistance to individuals with physical, intellectual, or developmental disabilities
  • Implement individualized service plans in collaboration with clients and care teams
  • Advocate for clients' rights, independence, and access to community resources
  • Assist with daily living activities, personal care, and mobility support
  • Maintain accurate documentation, case notes, and compliance records
  • Coordinate with healthcare providers, families, and community organizations
  • Monitor client progress and adjust support strategies accordingly
Requirements:
  • Experience in disability services, social services, or a related human services field preferred
  • Knowledge of person-centered care principles preferred
  • Strong communication, empathy, and interpersonal skills required
  • Ability to pass a background check and meet State licensing or certification requirements required
  • Valid driver's license, clear driving record, and reliable transportation required
  • Ability to work varied days and hours required
  • CPR/First Aid certification required (will provide if needed)
  • MAT: Wyoming Department of Health Medication Assistance certification required (will provide if needed)
  • High school diploma or equivalent preferred, but not required
