Description

New Alliance Counseling Services is seeking a dedicated Discharge Coordinator to join our Evaluation & Treatment (E&T) team in Colville, Washington. The E&T facility provides 24/7 inpatient psychiatric evaluation, stabilization, and treatment services for individuals experiencing acute behavioral health crises.

The Discharge Coordinator plays a critical role in helping patients successfully transition from inpatient care to community-based services. This position develops and coordinates individualized discharge plans, collaborates with interdisciplinary treatment teams, connects patients with housing, medical, behavioral health, and social service resources, and provides post-discharge follow-up to support recovery and reduce rehospitalization.

Key Responsibilities

  • Develop and coordinate comprehensive discharge and aftercare plans.
  • Facilitate referrals to community resources and treatment providers.
  • Coordinate care transitions with healthcare and social service partners.
  • Monitor patient engagement with aftercare services following discharge.
  • Maintain accurate clinical documentation and electronic health records.
  • Collaborate closely with psychiatrists, nurses, therapists, and community providers.


Qualifications

·        Bachelor's degree in a behavioral science field required.

·        Washington State Agency Affiliated Counselor credential required (or ability to obtain).

·        Two years of behavioral health experience preferred.

·        Strong communication, organization, and documentation skills.

·        Valid driver's license, proof of insurance, and satisfactory driving record.

·        Successful criminal background check required.
