Discharge Planner - FT

This position is responsible for discharge planning for the individual patient. Works within the Care Management to meet the discharge planning needs of patients, as well as a referral source for post-acute services including but not limited to home health, skilled nursing, physical rehabilitation, DME and counseling.

• Provides discharge planning for patients and families. Acts as a resource for available services to patients and families.
• Develops and maintain a good working rapport with personnel within the facility and outside community health, welfare, and social agencies to meet the needs of the patients and their families.
• Working knowledge of common disease entities, disorders and medical terminology to understand acute and ongoing needs of patients and families.
• Knowledge of health care systems and public programs such as those funded by Medicare, Medicaid, Medicare Advantage, Tricare (VA) and commercial insurance plans.
• Works in collaboration with nursing services as well as other departments to provide appropriate patient care.
• Develops and implements discharge planning in conjunction with the medical staff, nursing services and other departments which includes addressing the emotional, social, and physical needs of patients and their families.
• Maintains current knowledge of available health providers, programs, and resources.
• Assesses patient's available resources (i.e.: financial, family support, etc.)
• Assists patients and their families in locating and utilizing financial, legal, mental health, and other community resources.
• Uses knowledge of placement opportunities in area to assist patients in accessing the appropriate level of care (i.e.: Skilled Nursing, Rehab, Hospice, etc.)
