The role provides efficient, quality discharge planning services to patients and their families from initial intake to discharge, serving as a member of the interdisciplinary team supporting the organization's treatment program and philosophy and ensuring the delivery of quality treatment to patients and their families. The person participates in performance improvement activities, attends staff meetings, coordinates care through communication with physicians, nurse practitioners, clinical services, nursing, and intake department, and attends treatment team meetings as scheduled.Position Responsibilities Clinical / Technical Skills Evaluate patient care plans on a regular basis and update the plans when needed.Fax aftercare plans to referrals within 24 hours of discharge.Plan patient care in collaboration with all members of the healthcare team.Consult with other departments as appropriate to collaborate in patient care and performance improvement activities.Collaborate with other departments to identify operational problems and develop solutions.Work with all members of the healthcare team to maintain a collaborative approach in the care and treatment of the patient.Work closely with the social worker to integrate psychosocial management of patient/family needs.Attend daily treatment team meetings, coordinate care plans with patient, MD, therapist and RN.Initial discharge plan is formulated and noted on the psychosocial assessment form within 72 hours of admission.Assist the Utilization Management Department with all utilization activities as requested and directed.Participate in education on and implementation of clinical guidelines and protocols.Ensure documentation meets current standards and policies.Maintain optimal continuity of patient care through efficient planning, assessment, and coordination of healthcare services.Demonstrate flexibility, organization, and the ability to function under stressful situations.Maintain a good working relationship within the department and with other departments.Develop and coordinate an individualized discharge plan for the patient using treatment team and chart information to determine aftercare needs.Access family or community resources such as nursing homes, group homes, or private mental health practitioners to meet aftercare needs.Coordinate with team members to incorporate discharge planning in all phases of patient care by reviewing written progress notes and communicating any state or local agency legal requirement for referral or discharge planning.Document treatment provided to the patient in the medical record.Perform other duties as assigned.Safety Strive to create a safe, healing environment for patients and family members.Follow all safety rules while on the job.Report near misses, errors, and accidents promptly.Correct minor safety hazards.Communicate with peers and management regarding any hazards identified in the workplace.Attend all required safety programs and understand responsibilities related to general, department, and job-specific safety.Participate in quality projects as assigned and support quality initiatives.Support and maintain a culture of safety and quality.Teamwork Work well with others in a spirit of teamwork and cooperation.Respond willingly to colleagues and serve as an active part of the hospital team.Build collaborative relationships with patients, families, staff, and physicians.Retrieve, communicate, and present data and information both verbally and in writing as required.Demonstrate listening skills and the ability to express or exchange ideas through spoken and written communication.Adhere to the Standards of Behavior.Integrity Strive to always do the right thing for the patient, coworkers, and the hospital.Adhere to established standards, policies, procedures, protocols, and laws.Apply the mission and values of SUN Behavioral Health to personal practice and commit to service excellence.Support and demonstrate fiscal responsibility through supply usage, ordering of supplies, and conservation of facility resources.Complete required trainings within defined time periods, as established by job description, policies, or hospital leadership.Exemplify professionalism through good attendance and a positive attitude at all times.Maintain confidentiality of patient and staff information, following HIPAA and other privacy laws.Ensure proper documentation in all position activities, following federal and state guidelines.Compassion Demonstrate accountability for ensuring the highest quality patient care for patients.Show willingness to be accepting of those in need, and extend a helping hand.Desire to go above and beyond for others.Understand and accept cultural diversity and differences.Qualifications Education Required: Bachelors degree in a behavioral health field or business administration.
