Overview:

Department: Social Work

Shift: Full-Time; Days, 8-4:30pm

Facility: Mercy Medical Center

Location: Aurora, IL

 

Bilingual (Spanish/English) candidate preferred. 

Responsibilities:

Responsible for the coordination and scheduling of appointments. This individual will support both the Case Management team and Primary Care Group by coordinating follow-up appointments for patients prior to hospital discharge. Responsibilities will include scheduling appointments with our Primary Care Group and Multispecialty Group practices to help ensure continuity of care after discharge.

 

Qualifications:

EDUCATION, EXPERIENCE, TRAINING

Required qualifications

1.     Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions, in both managed care and non-managed care environments.

2.     Experience in basic to intermediate computer skills/knowledge.

3.     High School Diploma required.

 

Preferred qualifications:

 

1.     Current BCLS certificate required upon hire and maintain current preferred.

2.     Associates degree or LVN license preferred.

3.     Experience in the hospital setting preferred.
