Discharge Planning Assistant - Social Work - Westchester Hospital – Day Shift 

 

Across NewYork-Presbyterian/Westchester Hospital the interdisciplinary team approach to care coordination drives our quality outcomes and amazing patient stories. As a Discharge Planning Assistant, you’ll assist with facilitating successful discharge plans which are critical to length of stay reduction and patient flow.  

Join a team of Care Management, Social Work, and Nursing, in providing a high level of customer service to patients by ensuring a proper and timely discharge. Make regular rounds of all patients to assure that the thoughtfully crafted treatment plans are being implemented and communicated across the interdisciplinary team. Go above and beyond by securing resources for patients, contacting providers and community referrals to obtain authorizations, and setting up placement after discharge.  

This is a full time day shift opportunity, covering the hours of 9:00am –5:00pm, Tuesday – Saturday at NewYork-Presbyterian/Westchester Hospital in Bronxville; right across from the Metro North Station. 

Preferred Criteria 

  • Bachelor’s Degree 

Required Criteria 

  • High school diploma or GED 

  • 1 year of experience in a healthcare setting
