This position will serve in 1 East County: Cocke as a RISE (Recovery, Information, Support, and Engagement) Navigator. The position will facilitate SUD screenings and assessments, crisis intervention, solution-focused counseling, education and outreach, community partnerships, client linkage and support, documentation, and recovery support services. The position requires the ability to build relationships with individuals and community organizations, provide peer support, maintain appropriate boundaries, demonstrate cultural competency, and support individuals with dignity and respect.
Responsibilities
Facilitate SUD screenings and evidence-based mental health and substance use disorder assessments, receive referrals, and provide crisis intervention and solution-focused counseling.
Deliver education and outreach focused on preventing drug overdose deaths and communicable diseases such as Hepatitis C and HIV; distribute naloxone and overdose education materials.
Build and maintain partnerships with Syringe Service Providers (SSPs), Regional Overdose Prevention Specialists (ROPs), mental health agencies, treatment providers, and other community organizations.
Connect clients to community mental health, substance use treatment, harm reduction, primary health services, and recovery support services.
Complete detailed progress notes, referral tracking, and record keeping in compliance with contract, state, grant, and agency requirements.
Attend team meetings, case conferences, and training sessions and perform other related duties as required.
Travel is required with mileage reimbursement.
