HCT seeks an experienced nurse professional to serve as the Interim Disease Specific Care Coordinator for a 250+ bed hospital in California. This position largely focuses on coordination of the hospital's Stroke Program. Responsibilities include:
Leading and coordinating disease-specific certification programs (with a strong focus on Stroke), driving quality improvement initiatives, ensuring regulatory compliance, and serving as a clinical resource for physicians, nursing staff, and multidisciplinary teams.
The role focuses on improving patient outcomes through evidence-based practice, education, data analysis, and performance improvement while maintaining compliance with Joint Commission and other disease-specific certification standards.
Requirements:
Education: Bachelors Degree in Nursing preferred.
Experience: Previous program development and implementation experience preferred with a minimum of four (4) years of clinical nursing experience. Recent experience in acute clinical neurosciences preferred. Current knowledge of hospital regulations, patient rights and consent law. Thorough knowledge of TJC Disease-Specific and other regulatory requirements.
