Supports Coordinator (Early Intervention Services)Position SummaryUnder limited supervision of the Early Interventionist Supervisor, the Supports Coordinator (Service Coordinator/Family Trainer) provides ongoing service coordination for children (birth to age 6) with developmental delays or disabilities and their families. This role ensures families can access needed services, understand their rights, and receive coordinated, high-quality early intervention supports that promote child development and family capacity.Service Coordination & Case ManagementServe as primary point of contact for familiesAssist families in accessing needed servicesCoordinate and monitor service deliveryMake referrals and schedule appointmentsConduct follow-up to ensure services are providedAssessment & PlanningCoordinate evaluations and assessmentsFacilitate IFSP/FSP development and reviewsParticipate in multidisciplinary team meetingsHelp families identify goals and outcomesFamily Support & AdvocacyEducate families on rights and safeguardsPromote caregiver competence and empowermentSupport services in natural environmentsService MonitoringMonitor services to ensure alignment with IFSP/FSPCoordinate across service systemsSupport transition planningDocumentation & ComplianceMaintain accurate recordsComplete service notes and documentationEnsure compliance with Medicaid and IDEA Part CCollaborationWork with families, providers, and agenciesParticipate in team planning and communicationQualificationsBachelor's degree in one of the following fields or related area:Social Work, Psychology, SociologyEarly Childhood Education or Special EducationChild Development, Family Studies, or Health-related fieldCommunication Disorders or Rehabilitative CounselingMinimum 1 year of experience working with infants, toddlers, or children with disabilities (Alternative combinations of education and experience may apply).Preferred:Experience with Early Intervention, IDEA Part C, or pediatric servicesKnowledge of IFSP/FSP development and child developmental milestonesExperience working with families and community-based services Working Conditions- Community-based work - Flexible schedule - Travel requiredCare Lync is an equal opportunity employer and complies with the provisions of the Federal Civil Rights Act of 1964 and the Pennsylvania Human Relations Act of 1955 and all requirements imposed pursuant thereto.
